HTNG Platinum Membership Upgrade

Use this form to upgrade your existing Corporate membership in HTNG to Platinum. The cost is $2,400 per year starting in January 2007, with the first year prorated until the end of your HTNG membership year.

This is open to HTNG Corporate Members whose dues are paid up and who are not more than 30 days overdue on any payments to HTNG.

10% discount on conference registration for company employees.
Discounts on other sponsorships and events may vary.

PLEASE READ THE FOLLOWING TWO OPTIONS CAREFULLY, THEY ARE NOT THE USUAL YADA-YADA!

Payment Plan:

If my current membership expires after April 30, 2007, I elect to pay $800 now, $800 on April 1, and the balance on September 1 (but never more than my prorated total for 2007). Invoices will be sent on approximately February 1 and July 1. (Written agreement will be required.)

Upcoming Renewals:

If my current membership expires before March 31, 2007, please invoice me now for my renewal, so that we can pay it by January 31, 2007. Payment by January 31 is required if you will be applying for sponsorship or exhibit opportunities at the 2007 Members Meeting & Conference on March 5-7.

Authorization:

I agree to pay HTNG an amount equal to US$200 times the number of months between January 1, 2007 and our next renewal date. This fee is nonrefundable.

I understand that a billing invoice will be sent on or before 27 November 2006, or within 2 business days thereafter, to the e-mail address listed under billing contact (or the primary marketing contact, if no billing contact is listed). This invoice may be paid by check or bank transfer, or may be paid online using American Express, MasterCard, or Visa.

I understand that our Platinum membership status will become effective on the date when payment (or the first payment, if I have elected to defer a portion of the payment to 2007) is received.

	Marketing Committee
